County psychiatrist Dr. Maggie Belham (Paulina Porizkova) is called in to control the situation when it is discovered that a fearsome killer (Larry Drake) is on the loose in an abandoned asylum. Dr. Belham finds that the psychopath is a worthy adversary, especially when she discovers that she is lost in the hospital's maze of dark corridors. She has a potential helper in Quitz (Judd Nelson), the janitor, whose trustworthiness may hinge on his possible status as a forgotten inmate.
